Commitment Checklist and Acknowledgement *

Please read all requirements before completing the acknowledgement below:

We acknowledge commitment to Game On! is not a yearly option and understand that by accepting this offer, my student is committing to a 3 year extension program

We acknowledge the selection of Game On! is required to be selected as one of the 3 elective subjects in Years 8 and 9

We acknowledge my student is required to be enrolled in weekly instrument/vocal lessons on their primary instrument for the duration of the program. It is recommended students be enrolled in private tuition (Please Note: Some students may be required to be enrolled in private tuition. Please check your acceptance letter.)

We acknowledge my student is required to commit to at least one College ensemble for the duration of the program

We understand the Music Game On! Program includes the opportunities: including: free tuition on second study instrument; free second study instrument hire (where applicable) for the duration of the program; free theory tuition for the duration of the program; workshops and masterclasses with industry professionals; and performance and recital opportunities

We acknowledge students will sit a minimum Grade 2 Australian Music Education Board (AMEB) Theory of Music exam which will be invoiced at the current AMEB cost in Term 3 of Year 9

We acknowledge having read and understood the commitment requirements and on behalf of my student, we wish to accept the position offered in the Music: Game On! Program from 2025=2027

We acknowledge their is an annual levy; currently $150.00 for 2025, payable early in Term 1 annually for the duration of my student's participation in the program

Instrument/Voice Tuition *
